As part of the Community Care Team East, you will support adults in need of help to remain in their own homes.
Responsibilities
* Assess the needs of citizens and carers, plan, implement, review, and evaluate outcomes. Work with individuals to understand their unique circumstances and priorities.
* Champion the principles of choice and control, supporting people to make their own decisions and ensuring services are organised and risks managed in ways that are right for them.
* Maintain professional standards as the service develops, including developing and reviewing support plans and provision, and enabling the creative use of resources including assistive technology to ensure care is fit for the individual.
* Work closely with individuals and their families to promote well‑being and independence in line with best practice and the Social Services and Well-being (Wales) Act 2014.
* With the support of the team, use creativity and dedication to help individuals achieve their goals and reach their full potential.
Skills & Qualifications
* Registration with Social Care Wales to ensure the highest standards of professional practice.
* A diploma, degree or master’s in Social Work that showcases your knowledge and expertise in the field.
* Experience of multi‑disciplinary working to collaborate effectively with other professionals and deliver high‑quality care.
* Knowledge of the latest legislation, guidance and regulations in relation to Adult Services, enabling you to stay up‑to‑date with trends and best practices.
