Undertakes effective specialist initial assessment, risk assessment and rating scales to plan, implement and evaluate individualised care plans that are responsive to the needs of the service user. Able to work towards recovery model including effective discharge planning and WRAP plans. To facilitate in service user involvement / patient participation in area of responsibility. Utilise health promotion techniques to support service users in optimising their physical and mental health wellbeing.
Assess manage and evaluate risk as an ongoing process using the risk assessment identified by the Trust. Escalate risks and communicate effectively with senior staff. Accurate and up to date record keeping in keeping with NMC guidelines. Provide effective care and therapeutic interventions to service users presenting with a range of difficulties.
Work within competence to administer and medication as per route prescribed and document this clearly working within current NMC guidelines. Document and communicate medication changes. Act as patient advocate in all matters relating to patient welfare acknowledging and incorporating cultural preferences, health beliefs and behaviours. Competent decision making on highly complex issues without supervision is required.
