Job Summary
Social Worker required for work within BANES Discharge to Assess (D2A) Team. 37 hours a week with rates up to £35-38 per hour for experienced candidates. Contract minimum 3 months. Flexible, hybrid working.
The D2A pathway supports individuals to leave hospital safely and continue their assessment and recovery in the most appropriate setting, helping people regain independence and avoid unnecessary long-term care. The D2A team works closely with health, social care, therapy, and community partners to support safe hospital discharge and ongoing recovery within community settings. Team members play a key role in helping people regain independence and achieve the best possible outcomes following discharge from hospital.
Key Responsibilities
* Coordinate safe and timely hospital discharges
* Complete strengths‑based assessments within community settings
* Support individuals to maximise independence and recovery
* Work collaboratively with NHS, social care, therapy, and voluntary sector colleagues
* Develop person‑centred care and support plans
* Monitor outcomes and escalates concerns where appropriate
* Maintain accurate and timely case records
Qualifications & Requirements
* SWE registered
* Driver with access to own car
* Reside and work in the UK
