Position: Ambulatory Palliative Care Clinical Nurse Specialist (Band 6)

Site: Woodlands Hospice, Aintree Hospital Site (Liverpool)

Contract: Permanent, Part‑time – 22.5 hours per week, 3 days 09:00‑17:00 (Mon‑Fri). Flexible days off.

Salary: £38,682 – £46,580 Pro Rate Per Annum (Yearly)

Job Overview

We are seeking a dynamic Clinical Nurse Specialist to join the Ambulatory Specialist Palliative Care Team at Woodlands Hospice. Candidates must have previous palliative care experience and training specific to the role. You will be part of a team of Band 6 Clinical Nurse Specialists, working with the Specialist Palliative Care multi‑disciplinary team and wider hospital, hospice and community teams, caring for palliative patients and supporting them via Nurse‑Led Clinics.

Your role will involve managing your own caseload of palliative care patients in the community, triaging referrals and requests for advice and support, and contributing to the objectives of the clinical Palliative Care & Acute Oncology corporate objectives within the clinical governance framework.


Responsibilities

* Act as the clinical expert and professional resource to the community multidisciplinary team and to patients and carers.
* Participate in the rota as the Ambulatory Specialist Palliative Care Team triage coordinator within the IMPaCT Hub, triaging referrals, coordinating and responding to support requests.
* Carry responsibility for a defined caseload, offering specialist advice, setting care standards, assessing care needs and developing, implementing and evaluating care programmes.
* Conduct and coordinate independent holistic assessments and nurse‑led follow‑up clinics, using face‑to‑face, telephone and at‑home visits where appropriate.
* Support Hospice at Home service as Band 6 Clinical Coordinator in the event of absence, including on‑call rota participation and remote covers.
* Contribute to corporate objectives and work within the clinical governance framework.


Qualifications

* Registered Nurse / Midwife.
* Post‑basic specialist qualification relevant to speciality.
* Evidence of continued professional development.
* Recordable specialist qualification (desirable).
* Independent Nurse Prescribing (desirable).
* ECDL (desirable).


Experience

* Post registration experience.
* Extensive clinical experience.
* Involvement in audit.
* Participation in research (desirable).
* Evidence of leading and facilitating change, audit and change management ability (desirable).


Knowledge

* In‑depth specialist knowledge.


Skills

* Teaching / assessment skills, organisation and negotiation skills.
* Effective communicator.
* Root Cause Analysis.
* Leadership and motivation skills.
* Ability to motivate self and others.
* Computer literate.
* Work independently and within a team.
* Diplomatic, assertive, confident and recognise own limitations.
* Able to travel between sites, flexible.


Additional Requirements

Must have appropriate UK professional registration.
