The Support Broker provides personalised support to individuals, helping them access care, resources, and community connections to maintain independence. This role involves working with clients, professionals and organisations to coordinate and deliver tailored solutions that address health and social care needs, independence, choice, and control.
Key Responsibilities
Client Support
* Work with individuals to design and implement personalised care plans.
* Advocate for clients with external organisations to ensure needs are met.
* Maintain regular communication with clients, progressing cases from referral to closure.
* Facilitate community integration and sustainability of support for clients.
Coordination and Administration
* Manage a caseload of 30-60 clients, adapting to changing priorities.
* Attend and represent the organisation in multidisciplinary team meetings.
* Use a case management system to document client interactions and monitor progress.
* Coordinate services such as housing support, welfare benefits, and independent living aids.
* Build relationships with internal and external partners, including VCSE organisations, statutory services, and care providers.
* Ensure seamless communication and referrals between stakeholders to meet client needs effectively.
Problem‑Solving
* Identify creative, alternative solutions to traditional care models.
* Leverage available resources like grants and funded services to enhance client outcomes.
Essential Skills and Attributes
* Organisation & Time Management: Ability to manage multiple tasks, prioritise effectively, and adapt to changes.
* Communication: Strong verbal and written skills to liaise with clients, teams, and external organisations.
* Empathy & Sensitivity: Demonstrates compassion and understanding towards diverse client needs.
* Technology Proficiency: Experienced with Microsoft Office and case management systems.
* Team Player: Collaborative and supportive in multidisciplinary settings.
