An exciting and new opportunity has arisen for a Care Coordinator to work within Early Intervention in Psychosis Team in Bolton.

We are looking for an experienced and enthusiastic Registered Mental Health Nurse who has a real passion for providing support at the first episode of psychosis to promote mental and physical heath, wellbeing and recovery.

The purpose of the role is to reduce the stigma associated with psychosis, provide an early intervention to reduce the length of time that people remain undiagnosed and untreated and to offer support and guidance throughout the persons recovery journey.

You will be joining a client centred, dynamic and friendly team, who prides themselves on delivering high quality, evidence-based interventions for those experiencing first episode psychosis.

Main duties of the job

Your role will include providing effective care coordination to an identified caseload of complex service users, ensuring the delivery of evidence based interventions to encourage successful outcomes under the Care Programme Approach (CPA).

You will support the wider team in duty and assessment tasks and will demonstrate effective communicate with clients, carers and other internal and external agencies.

Education/Qualification

Essential criteria

8. A qualification in Mental Health Nursing (RMN)
9. Registration with the regulatory body as applicable; NMC

Desirable criteria

10. Evidence of robust continued professional development including training in relevant subjects.

Experience

11. Demonstrable experience of working with people with psychosis.

Desirable criteria

12. Experience of working with people with complex presentations and high rates of co-morbidity.
13. Experience of work within a multidisciplinary team.
14. Experience of working within a EIT/CMHT setting; managing a caseload of service completing assessments with people who have been referred into the and

Knowledge

15. An understanding of the needs and difficulties of people with severe and enduring mental health difficulties
16. Knowledge and understanding of the Care Program Approach 2008.
17. Knowledge and understanding of The care Act.
18. Knowledge and understanding of NICE Guidelines in relation to First Episode Psychosis.

27. Be able to contribute to the planning delivering and monitoring of interventions and treatments including outcome measure assessment tools and audits.

Essential criteria

28. There is a requirement to travel mainly within the Bolton area and an occasional requirement to travel across the wide footprint of the Trust to attend meetings and events relevant to the role.

31. Ability to accept and use supervision appropriately and effectively

The Trust mandates all post holders who require a DBS for their role to subscribe to the DBS Update Service. You will be required to enrol for this service for a fee of £16 per year and maintain registration throughout your employment with GMMH.
