Job Overview
The UCR Service provides urgent support, seven days a week, to help prevent unnecessary hospital admissions. This is achieved by delivering a two-hour crisis response by a multi-skilled team of professionals at home or at the patient’s usual residence. The team also provides a general virtual ward service supported by digital technology and remote monitoring, enabling patients to receive hospital-level care at home safely and in familiar surroundings.
Our multidisciplinary teams – consisting of GPs, nurses, paramedics, pharmacists, occupational therapists, physiotherapists and health & therapy assistants – work flexibly to deliver a 7‑day, 365‑day service between 8 am and 8 pm. The service includes clinical interventions such as intravenous therapy and care pathways for admission avoidance.
Responsibilities
* Deliver high-quality community care, supporting patients toward rehabilitation, self-management and independence throughout the continuum of care.
* Participate in multidisciplinary teams to provide evidence-based care and facilitate staff learning and development.
* Act as a practice supervisor for junior staff and students.
* Conduct safe and effective reviews and referrals to the appropriate multidisciplinary team member.
* Delegate and prioritise workload effectively.
* Maintain professional development to keep competence in practice, as negotiated in supervision.
* Identify health, safety and security risks and manage them appropriately.
* Engage in service provision and contribute to change initiatives.
Qualifications
* Experienced nurse or paramedic with a keen interest in developing holistic assessment skills and knowledge of urgent community nursing.
Requirements
* Full UK driving licence and access to a vehicle, due to the large geographic area covered by the team.
