Support Worker - Assisted Discharge Service – Gloucestershire
Location: Gloucestershire Royal Hospital, GL1 3NN
Salary: £22,932 per annum (pro‑rata for under 35 hours). Job Share Available
Hours: Various hours available: shifts 10am–8pm weekdays and 10am–5pm weekends (one weekend per month)
Contract: Permanent
Driving Requirement: Full UK Driving Licence held for a minimum of 2 years
Overview: When someone is ready to leave hospital but has no one to help them get home or settle in, you come in. As a Support Worker in our Assisted Discharge Service, you will help vulnerable adults return home safely and comfortably. You will assess their needs, ensure their home is ready, and provide short‑term support to reduce the risk of readmission.
What a day looks like:
* Transport patients home and ensure the environment is safe and comfortable.
* Collect prescriptions, check food supplies, and complete light household tasks.
* Offer emotional support and guidance, helping service users regain independence.
* Liaise with hospital staff and respond to referrals from clinicians.
Note: This role does not involve personal care (e.g. bathing, cleaning, or administering medication).
Requirements for success:
* The Right to Work in the UK (sponsorship not offered).
* A full UK manual driving licence and access to a vehicle.
* Understanding of NHS and Social Care services.
* Professionalism, discretion, and flexibility to work outside standard hours.
