You will play a vital part in improving the lives of our patients by coordinating services, working closely with GPs and the wider Primary Care Network (PCN) team. You will be the link that ensures every patient receives timely, person-centred care, making sure no one falls through the cracks.
Main duties of the job
Key Responsibilities / Job Description
* Coordinate referrals from GPs and clinical staff, linking patients with the right support such as Social Prescribers, the Wellbeing Hub, or community agencies. Follow up to ensure services are delivered effectively.
* Work closely with the multidisciplinary team and external partners to streamline care and share information.
* Support patients with accessing services, including making appointments and arranging transport where necessary.
* Help patients complete healthcare-related paperwork and navigate social care systems.
* Keep accurate, up-to-date records of patient interactions using clinical systems, contributing to data collection and audits.
* Attend and contribute to multidisciplinary team (MDT) meetings, including minute taking when required.
* Support vaccination and health promotion clinics, and assist with Quality and Outcomes Framework (QOF) and other enhanced services.
* Actively listen to patients and carers, collecting feedback and sharing insights to improve care delivery.
* Identify and support individuals who would benefit from personalised care planning, in collaboration with the clinical team.

Person Specification
Qualifications
* Good general education (minimum GCSE level or equivalent)
Experience
* Experience in a people-facing role e.g., customer service, patient care, community services
* Experience in healthcare settings e.g., NHS, GP Practice, Primary Care
* Familiarity with clinical or patient administration systems
