The post holder, as part of a multidisciplinary clinical team, is responsible for delivering specialist nursing care to patients with diabetes, their carers, and health professionals in secondary care.
Key responsibilities include liaising with other healthcare professionals within the Trust and in primary care to ensure patient safety during inpatient stays and upon discharge, managing a caseload of patients with T1DM and T2DM, and working autonomously within the specialist area.
The role involves promoting effective communication between primary and secondary care, supporting development of diabetes services, and participating in audits and guideline development. Qualified independent prescribers will undertake medication prescribing responsibilities.
The post also includes organizing educational programs, supporting multidisciplinary policy development, and evaluating service effectiveness.
