Paying up to £38 an hour

* To work as part of a friendly, innovative and supportive team and be involved in providing a responsive, equitable and high quality community occupational therapy service to adults with a long term condition in Buckinghamshire.
* To undertake comprehensive and holistic assessments of adults with long term conditions and their carers (formal/informal) within the home environment.
* To be responsible for organising and managing your own caseload; to ensure assessments and reviews are performed and documented in a timely way.
* To develop good understanding of the important role of equipment, minor and major adaptations and appropriate housing play in enabling and promoting functional independence. Develop understanding of the legislation and criteria that inform our practice.
* To provide support and input to the OT assessment clinics across the county.
* To develop competent moving and handling skills and learn techniques to enable single-handed care.
* To contribute to case resolution through input to the OT Triage function. To gain knowledge and experience of how referrals are managed from the first point of contact and how innovative interventions are used to reduce risk, meet clients’ needs and effectively manage the demands on our service.
* To develop and consolidate areas of clinical interest, input into project work focused on service development and improvement, audits and reviews, evidence-based practice and raising the profile of community OT within adult social care and wider community.
* To engage in regular supervision and ongoing professional development; to have good self-awareness and be able to identify own limitations and training needs and apply this to practice.
* To be responsible for supervising appropriate staff across the integrated
