Detailed job description
and main responsibilities
1. To implement and deliver patient care and treatment plans and support the Registered Nurse in assessment and evaluation aspects of patient care plans,in accordance with Trust policies, procedures and standards and that fully reflect the Poole Approach
2. To participate in the supervision and assessment of healthcare assistants within their own practice setting
3. To support and may participate in the administration of audit and practice developments/initiatives within their practice setting
4. The post-holder has the ability to work without direct supervision, at times delivering care and undertaking defined interventions independently, in line with the patient’s plan of care, as agreed by a Registered Nurse.
5. The post-holder is expected to work under the direction and indirect or remote supervision of a Registered Nurse within their defined practice setting.
6. The post-holder is responsible and accountable for their own practice, working within the defined limits of competence and within the scope of the role

Qualifications
Essential criteria
7. GCSE or equivalent Maths and English
8. Nursing Associate foundation degree in health and social care
9. Willingness to attend appropriate study sessions & courses to complete the required competencies of the assistant practitioner role
10. Knowledge and understanding of the nursing process
Desirable criteria
11. Knowledge and understanding of the Mental Capacity Act and Deprivation of liberties and other key relevant legislation
12. Knowledge of specialist area of care
Experience
Essential criteria
13. Experience of working in a caring environment
14. Experience of working in a multi-disciplinary team
15. Experience of delivering individual plans of care
Desirable criteria
16. Experience of working in an acute hospital within the last 2 years
17. Experience of working to deadlines and prioritising care delivery

31. Assess the needs and plan the care of patients, evaluating progress to ensure the care remains appropriate and facilitate colleagues in these processes.
32. Evidence based theoretical knowledge in wound care in order to apply to practice.
33. Good understanding of the deteriorating patient and instigating appropriate treatment.
34. Comprehensive applied anatomy and physiology to practice.
35. Good knowledge of NMC standards and limitations of practice.
36. Knowledge of Clinical governance.
37. Knowledge and understanding of Trust values.
