The purpose of this role is to assess and prioritise all new external referrals to Ashgate Hospice, ensuring patients are directed to the appropriate service promptly and efficiently. Working closely with the Lead Clinical Assessor, you will allocate referrals, contact patients and referrers to establish urgency, and ensure efficient recording of outcomes.
You will also provide professional advice and guidance to patients, families, and healthcare professionals, signposting them to appropriate services as required. The post holder will have excellent communication and documentation skills and be a motivated and flexible worker. The post holder will work closely with the current SinglePoint team and wider team members within the organisation, to continue to develop the SinglePoint of Access service.
Key responsibilities include:
1. Assess referrals to determine urgency and priority, ensuring timely responses for patients and referrers.
2. Provide expert advice and support to referrers, patients, and families.
3. Collaborate with the lead clinical assessor in shaping the SinglePoint assessment process and policy.
4. Contribute to the ongoing development of the service through a quality improvement approach.
5. Work closely with patient-facing teams to streamline care pathways and ensure efficient service delivery.
6. Develop strong working relationships with primary care professionals and other referrers to enhance referral processes and feedback mechanisms.
7. Provide cover for the lead clinical assessor during periods of annual leave.
8. Foster effective communication and collaboration with community professionals, hospitals, voluntary organisations, and other hospices.
