We are seeking a dedicated and compassionate Adult's Social Worker to join our Hospital Team in Bath and North East Somerset. This role is pivotal in ensuring that vulnerable adults receive the support they need during their hospital stay and upon discharge. As part of a multidisciplinary team, you will work collaboratively with healthcare professionals, families, and community services to facilitate safe and effective transitions for patients.
Your key responsibilities will include:
* Assessing the social care needs of patients and developing tailored care plans.
* Conducting risk assessments and safeguarding investigations as necessary.
* Providing emotional support and guidance to patients and their families.
* Coordinating with external agencies to ensure continuity of care post-discharge.
* Maintaining accurate and up-to-date records in compliance with legal and organizational standards.
To be considered for this role, you must possess a recognized social work qualification (e.g., Degree in Social Work or equivalent) and be registered with Social Work England. Essential skills include excellent communication and interpersonal abilities, strong analytical and problem-solving skills, and a deep understanding of adult safeguarding and mental capacity legislation. Experience in a hospital or healthcare setting is highly desirable.
