What you will do: * Provide specialist pharmacy technician services to the Welney Ward at Princess of Wales Hospital in Ely, supporting the pharmacist in delivering clinical pharmacy services across various specialities including elderly care, rehabilitation, stroke, and palliative care. * Conduct medicines reconciliation and assess patients' own medicines upon their admission, solving any issues and documenting them precisely on electronic patient record systems. * Order necessary medicines both for inpatients and ward stock, ensuring a seamless supply. * Participate in weekly multidisciplinary team meetings to support patient care during their stay and assist in their discharge planning. * Engage with patients and nursing staff in medication self-administration programmes to foster patient rehabilitation and independence. * Collaborate with community pharmacies to ensure smooth transitions upon patient admission and discharge. Requirements: * A minimum of 2 years' experience as a trained Pharmacy Technician. * HCPC registration required. * A car driver is preferable to facilitate travel across campuses and community pharmacies.
