Job overview

You will be a key member of Hub Admin Service and support other members of the Integrated Discharge Service by providing administrative support to a high standard relating to the Trust’s patient capacity and discharge processes.

You will need to have excellent IT and organisational skills to support the daily work of the service, ensuring referrals are processed in a timely manner and daily information reports to support the Integrated Discharge Service are made available to senior management colleagues in accord with agreed deadlines.

Main duties of the job

Your great office and IT skills will be essential and you’ll need to use your initiative, but you will also have a supportive team around you to help.

You will be expected to communicate with a range of staff and external agencies using a variety of communication methods including, verbal, written, interpersonal and electronic, circulating robust information therefore your strong communication and organisational skills will come into play as you manage your workload and ensure tasks are completed timely and any concerns escalated.

Detailed job description and main responsibilities

Your main jobs and responsibilities will be;

6. To manage and update central information management systems within the Integrated Discharge Service
7. Manage a central repository of information to support the Integrated Discharge Service in a timely and confidential manner.
8. Update of referrals onto management information systems relating to patient details, needs and resources in a timely and accurate manner.
9. Act as a central point for the Trust where ‘live’ information can be gathered on capacity across the two sites.
10. Produce and circulate robust daily information reports to support the Integrated Discharge Service processes and make available in a timely fashion to senior management colleagues.
11. Actively develop effective internal and external relationships and work closely with colleagues to ensure a successful service delivery.
12. Communicate with clinical and social care colleagues within the Integrated Discharge Service involved with each patient using a variety of communication methods including, verbal, written, interpersonal and computer, and passing information in a format that is appropriate to the individual request.
13. Respond to external and internal telephone enquiries from staff and others in an efficient, tactful and helpful manner and ensure that any messages/information are passed to the appropriate person in a timely manner.
14. Contribute to the overall effectiveness and development of the Integrated Discharge Service, the Division, the Trust and other partner organisations.
15. Plan own workload in conjunction with Hub Manager to ensure continuity of service and support.
16. To personally contribute to the overall effectiveness and development of the Integrated Discharge Service to ensure the delivery of safe and effective discharge processes.
17. Take responsibility for one’s own continuing professional development, including keeping up to date with activities of the wider Service.

Qualifications and Education

Essential criteria

18. Good General Education (GCSEs)
19. Business and Administration qualification or significant relevant experience

Desirable criteria

20. NVQ 3 information/Administration

Knowledge and Experience

Essential criteria

21. Advanced IT Skills in MS Office applications (Word, Excel, Access, Outlook)
22. Experience of using varied communication styles in an administrative setting
23. Previous experience of management information systems (updating, extracting, and producing reports)
24. Significant administrative experience working in an office/business setting

Desirable criteria

25. Previous hospital or social care administration experience
26. Use of Electronic Patient Record systems
27. Knowledge of discharge pathways
