Groves Health is seeking an experienced Practice Nurse to provide high-quality care to patients at Hinchley Wood Surgery. The role may require work at other sites (Richmond, Wimbledon, New Malden) from time to time. The position is available 4 days a week with hours by arrangement.

Job Description

Primary Responsibilities
* Identify, manage, and support patients at risk of developing long-term conditions
* Provide routine nursing care to patients in accordance with clinical evidence, NICE guidelines, and the NSF
* Provide vaccination and travel medicine services
* Provide wound care (ulcers, Doppler, etc.)
* Request pathology services as necessary
* Provide chronic disease clinics, deliver patient care, and refer to secondary/specialist care as required
* Maintain accurate clinical records in line with extant legislation
* Ensure SNOMED codes are used effectively
* Maintain chronic disease registers and vaccination programmes for adults and children
* Provide cervical cytology and monitor vaccination targets
* Prioritise health issues and intervene appropriately
* Assess, refer, and support patients presenting with mental health needs
* Support patients in using prescribed medicines or over-the-counter medicines within scope of practice
* Support safeguarding matters in line with local and national policies
* Deliver opportunistic health promotion where appropriate
* Attend clinical and practice meetings and clinical supervision
Secondary Responsibilities
* Develop, implement, and embed health promotion and well-being programmes
* Implement and evaluate individual treatment plans for chronic disease patients
* Assist GPs with procedures when required
* Chaperone patients where necessary
* Support the team in dealing with clinical emergencies
* Liaise with external services/agencies to support vulnerable patients
* Delegate clinical responsibilities appropriately within scope of practice
* Understand local policies for substance abuse and refer patients appropriately
* Act as audit lead and utilise the audit cycle
* Monitor safe storage, rotation, and disposal of medicines
* Support junior nursing staff and participate in local initiatives and shared learning
* Review clinical practices in response to national policies and initiatives
Experience and Qualifications
Experience
* Essential: 2 years' relevant experience in a primary care environment within the last 2 years
* Desirable: Experience in chronic disease management
Qualifications
* Essential: Nursing degree or equivalent; Registered Nurse (NMC)
* Evidence of immunisation training (childhood, adult, travel)
* Experience in complex wound dressings/leg ulcer management
* Evidence of cervical cytology training
* Desirable: Standards for Students Supervision & Assessment (SSSA); Diabetes Diploma; Family Planning qualification
Clinical Knowledge & Skills
Essential
* Immunisations (flu, routine, childhood, B12, travel)
* Wound care and management of small injuries; removal of sutures and staples
* Women’s health (cervical cytology, contraception, etc.)
* Dressings; diabetes review; hypertension; cardiovascular disease – primary prevention
* Coronary heart disease; accurate clinical notes; scope of practice with GP referral
* NICE guidelines and extended brief interventions understanding
* Cytology certificates; travel medicine; ECGs; venepuncture; asthma review
* PGDs and related policies; chaperone procedures
* Desirable: Knowledge of mental health conditions; pathology test requests and result processing; evidence-based practice; public health awareness; health promotion strategies
