Join the Home First Therapy Team in Buckinghamshire
An exciting opportunity has arisen to become part of the Home First Therapy team in Buckinghamshire. As the team is still developing into a substantive service, team members will have the opportunity to influence and shape its growth.
We are seeking motivated and skilled individuals to support patients recently discharged from hospital, helping them regain independence and remain in their own homes.
Role Responsibilities
1. Take a holistic and person-centred approach to support patients' recovery from hospital admission and achieve their short-term goals.
2. Visit patients at home to perform assessments and treatments for those with complex needs within expected timescales.
3. Provide advice, guidance, and information to health and social care professionals, carers, relatives, and other non-professionals.
4. Undertake all clinical duties as an autonomous practitioner.
5. Participate in professional and service development activities and contribute to organizational objectives.
