The Role
The Onward Care Team is based at Worcester Royal Hospital and the Alexandra Hospital operating a 7-day service on a rota basis. The team is a multi-disciplinary team consisting of Worcestershire County Council and Health and Care Trust staff.
The team work closely with our acute colleagues and reablement team with a focus on the home first principle, strengths-based practice, and collaborative working, utilising the 3 Conversation model and working within the Care Act.
The team is committed to working with patients to establish their goals and aspirations and work towards returning patients to their own homes for as long as possible. Working with the Mental Capacity Act to ensure least restrictive options and patient choice is at the heart of discharge.
As a Social Care Worker, you will be expected to manage your own cases supporting hospital discharge, identifying any Adults who may be at risk of abuse or neglect. For more complex work or Safeguarding cases, you will then work alongside a qualified Social Worker.
The Onward Care Team is a supportive team who offer regular supervision and reflection to develop your knowledge, skills, and practice.
