Were looking for a Complex Care Manager to join our dedicated team at our specialist Adult Complex Care and Dementia Nursing Home.
Youll play a key clinical leadership role overseeing care quality, developing personalised care plans, and supporting our staff team to deliver exceptional, person-centred care.
What Youll Do
- Lead clinical assessments and manage complex care packages.
- Oversee and review personalised care plans for adults with dementia and complex needs.
- Support, mentor, and develop nursing and care teams.
- Ensure care delivery meets CQC standards and best practice guidelines.
- Work collaboratively with families, GPs, and multidisciplinary teams.
What Were Looking For
- Registered Nurse (Adult) NMC Registered.
- 2+ years experience in complex care, dementia care, or nursing home management.
- Strong leadership, communication, and clinical assessment skills. xsngvjr
