Job overview The Specialist Doctor in Senior Adult Medical Services (SAMS) will deliver high-quality, patient-centred care for older adults across acute and community settings. The role includes comprehensive ward-based care, surgical and frailty in-reach, frailty cover within the Emergency Department, and participation in outpatient clinics. The postholder will work collaboratively with multidisciplinary teams to ensure timely assessment, effective treatment, robust discharge planning, and optimal patient flow across the pathway of care. The role includes participation in the General Internal Medicine (GIM) on-call rota, provision of prospective cover for colleagues, and dedicated Supporting Professional Activities (SPA). SPA time will support teaching and supervision, audit, research, clinical governance, quality improvement, and service development. Key service objectives include the delivery of seven-day frailty services, rapid assessment through the OPSSU, expansion of the iOPAL service, and strengthened integration with community services via Locality Hubs and Virtual Wards. The postholder will also contribute to clinical leadership, service transformation, and quality improvement initiatives aimed at improving patient outcomes, enhancing system resilience, and supporting a positive and inclusive workplace culture. Advert Clinical Duties & Patient Care Responsibilities: Deliver specialist doctor-level clinical services across wards, surgical and frailty in-reach, Emergency Department, and outpatient areas. Provide expert diagnosis, investigation, treatment, and ongoing management in accordance with Trust policies and best practice. Maintain continuing responsibility for patient care, including clinical administration across a 7-day service model. Participate in the General Internal Medicine (GIM) on-call rota and prospective cover for colleagues to ensure service continuity. Collaborate with Hub GPs, A&E consultants, and community teams to support integrated patient pathways. Assist in the development, use, and teaching of SAMS pathways to ensure safe and efficient patient flow. Administrative, Leadership & Professional Responsibilities: Provide clinical leadership, support service development, audit, research, and quality improvement initiatives. Engage with medical, nursing, and operational colleagues to deliver departmental objectives and improve patient experience. Undertake administrative duties, attend meetings, support appraisal, job planning, and rota organisation. Participate in formal and post-graduate teaching, training, and educational activities. Develop special interests and research aligned with departmental objectives and provide learning opportunities for staff. For more information about a career at ASPH please visit: www.asph-careers.org Detailed job description and main responsibilities Clinical Duties & Patient Care Responsibilities · Deliver clinical services of the highest professional standard at consultant level within the Department. · Work collaboratively with consultant colleagues and the multi-disciplinary team (MDT) to provide a high-quality, safe, and well-led service for patients at Ashford & St Peter’s Hospitals. · Provide expert diagnosis, investigation, treatment, and ongoing management of patients in accordance with Trust policies, national guidance, and best clinical practice. · Deliver Direct Clinical Care (DCC) across all areas of the Department, supporting senior clinical decision-making and sharing equitably in the clinical workload. · Provide comprehensive senior clinical cover to the Department, ensuring patient safety and continuity of care. · Maintain continuing responsibility for patients under their care, including associated clinical administration, across a 7-day service model. · Contribute, alongside consultant colleagues, to 7-day consultant ward cover at St Peter’s Hospital. · Participate in the General Internal Medicine (GIM) on-call rota during rotations at St Peter’s Hospital. · Work closely with consultant colleagues in A&E to support the effective management of acute medical emergencies. · Engage fully in the care of patients outside the Department, providing timely specialist advice and opinions when requested by other specialties. · Work with Hub GPs within the Northwest Surrey Locality Hubs to support integrated patient care. · Assist in the development, use, and teaching of SAMS pathways, supporting safe and efficient patient flow. · Collaborate with CSH Surrey colleagues to ensure delivery of agreed performance targets and service objectives. · Provide prospective cross-cover for consultant colleagues to ensure continuity of Geriatric Medicine services across Ashford & St Peter’s Hospital and CSH Surrey community hospital wards during periods of leave. · Ensure safe and effective handover of care at all times. Administrative, Leadership & Professional Responsibilities · Engage proactively with medical, nursing, operational, and managerial colleagues to deliver departmental objectives, including improved patient experience and high-quality clinical outcomes. · provide clinical leadership within the Department, supporting service development and quality improvement initiatives. · Actively participate in education, training, audit, research, and clinical governance activities across the multi-disciplinary workforce. · Support the delivery of Trust priorities, national standards, and transformation programmes. · Undertake administrative duties associated with patient care and departmental responsibilities in a timely and effective manner. · Participate flexibly in service delivery, with any changes to duties agreed through negotiation and reflected in team job plans. · Undertake any other duties appropriate to the role of a consultant in line with Trust requirements.
