The Community Support Service is offering an exciting opportunity for a 1‑year fixed term Assessment Officer to join our Outstanding CQC‑rated reablement team.
We support people to regain and maintain their independence through short‑term recovery‑focused reablement. Our approach empowers individuals to rebuild skills and confidence after illness or injury, enabling them to live as independently as possible. We work collaboratively with health partners, the voluntary sector, occupational therapy, and social work to deliver a fully integrated service.
ABOUT THE ROLE
* 30 hours per week, working on a rota including weekends and bank holidays
* You will complete strength‑based assessments, enabling customers to set their own goals and shape their personal reablement plan
* You will work alongside a dedicated team of Community Support Workers to support customers with medication, self‑care, confidence building, and accessing community resources
* The role plays a key part in supporting people returning home from hospital and maintaining independence in the community
WHAT YOU’LL NEED
* Strong communication and assessment skills
* Full driving licence and access to a car for work

You will need to have a full driving license and access to a car when at work.
