Qualified Social Worker, Hospital Team
Pay rate £32.55 per hour

Vitalis are recruiting for an experienced Social Worker to work in a hospital team in Westminster Council.
Key Responsibilities:
1. Triaging referrals on behalf of the Tri-Borough Hospital Discharge Team
2. Completing quick assessments for residents of the Tri-Borough
3. Undertaking Care Act Assessments and developing Support Plans
4. Completing Safeguarding Concern Assessments (Section 42.1)
5. Setting up care packages to facilitate safe discharge
6. Providing information and signposting to A&E staff
Essential Requirements:
7. Social Work England Registration
8. Proven experience in hospital discharge work
9. Strong knowledge of the Care Act and safeguarding procedures
10. Ability to work in a fast-paced environment and manage competing priorities
