Are you a dedicated and compassionate Nurse Practitioner looking to make a real difference in primary care? We are seeking an experienced and proactive Nurse Practitioner to join our supportive and forward-thinking GP surgery team.
About the Role:
In this rewarding role, you will work autonomously and as part of a dynamic multidisciplinary team to deliver high-quality, person-centred care to our patients. Key responsibilities include:
* Providing advanced clinical assessments, diagnoses, and treatment plans for patients with acute conditions
* Undertaking independent consultations, prescribing in line with policy and local guidelines, and making appropriate referrals
* Supporting the proactive management of chronic disease patients in line with QOF and best practice
* Participating in clinical audits, quality improvement projects, and regular MDT meetings

* Strong collaborative skills to work alongside GPs, nurses, pharmacists, social prescribers, and administrative staff
