The service provides person centred support to those with complex mental health needs to develop their skills to live more independently in the community. What youll do

* Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans.
* Sporting customers with independent living skills such as; budgeting, maximising customers income, cooking, engaging with external services.

* Promoting Home Groups values within your team.
* Work in close collaboration with; local authority, NHS mental health teams, safeguarding teams and housing option teams.
