Role Overview
CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! We are a dynamic and forward thinking team looking for a band 6 nurse to join our established Watford and Three Rivers community nursing team as a Deputy Team Lead. A great opportunity has arisen for an experienced and motivated registered nurse to joint lead one of our neighbourhood teams under the direction and leadership of the Team leaders. Your role would be varied and interesting carrying the responsibility for a patient caseload which involves the assessment, planning, implementation and evaluation of evidence based care to patients in their home environment. Previous community experience is essential.

The successful applicant must be someone with a strong commitment to working as part of a team, be approachable, a good communicator, have the ability to work autonomously and be receptive to change.
Responsibilities
* To set and maintain standards of care and deploy, coordinate and supervise the nursing team and learners as required.
* To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
* To be responsible for the team’s assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-8pm.
* To be responsible for the team’s maintenance of patient and other records.
* To participate in the long term management of patients within the locality.
* To have continued responsibility for a defined caseload.
