Detailed Job Description And Main Responsibilities Patients referred from General Practitioners, the Emergency Department, South Central Ambulance Service and Urgent Care Response teams are initially managed in Same Day Emergency Care or the Acute Medical Unit (3 linked ward areas). Medicine for Older People provide the service for patients over the age of 80 years. The team works closely with its community counterparts to ensure seamless care. You will supervise inpatient care of approximately 10 – 25 patients supported by a senior nurse/allied health professional case manager and trainee junior medical staff. The medicine for older person unit has a dedicated bed compliment of 140 beds over 7 ward areas, although patients may sometimes be located elsewhere in the hospital. The locality model of care has been refined over many years leading to strong community links and excellent continuity of care for patients and their carers. In order to deliver comprehensive care across boundaries, there are strong working relationships between the acute hospital and community providers of health and social care. Our expertise in caring for older people is recognised and we are closely involved in developing appropriate services and relationships by commissioners.
