Salary: £15.66 per hour

Hours: Full time – 37 hours per week (Monday–Friday, 08:30–17:00)
About the Role
Join Nottinghamshire County Council’s Adults Social Care & Health team as a Social Work Assistant, where you’ll support the delivery of person-centred care through assessment, support planning, and signposting to community resources. This role plays a vital part in promoting independence, safeguarding vulnerable adults, and helping individuals to live fulfilling lives within their communities.
This vacancy has arisen due to ongoing recruitment challenges, and the Council is looking for someone who can step in and contribute quickly and effectively to team priorities.
Key Responsibilities
Assess and review the needs of individuals and carers, coordinating interventions to achieve personalised outcomes.
Promote reablement and ensure access to relevant services.
Identify and promote community assets, preventative services, and alternative funding options.
Deliver information, advice, and support to service users and their carers.
Support safeguarding enquiries under the direction of safeguarding officers or managers.
Manage a social care caseload independently, with oversight from supervisors.
Liaise with service providers and community organisations to ensure quality support.
Participate in service development initiatives and flexible team duties as required.
Apply the Mental Capacity Act where necessary.
Accurately maintain records and contribute to assessments, including continuing healthcare.Experience & Knowledge
At least 2 years of relevant experience (paid, personal, or voluntary).
Understanding of adult community care, reablement, safeguarding, and the Care Act.
Experience working in or with multi-agency teams and liaising with service providers.
Familiarity with current Adult Social Care policies and frameworks.
Experience using electronic case management systems.Skills & Competencies
Strong interpersonal and negotiation skills.
Ability to assess needs, solve problems, and prioritise caseloads.
Confidence in working independently and as part of a team.
Excellent written and verbal communication.
IT proficient with strong record-keeping and reporting skills.
Committed to customer care and professional development.Additional Requirements
NVQ Level 3 in Health and Social Care (or equivalent), or evidence of relevant experience.
Full UK driving licence and access to transport (reasonable adjustments considered).
Willingness to engage in supervision, training, and ongoing service improvement.
Commitment to safeguarding, equality, and information governance policies.
