Position Details:
* Location: Warren, MN
* Schedule: Full-Time or Part-Time | Day, Evening, or Overnight Shifts | Includes Every Other Weekend

Your Role and Responsibilities
As an LPN, you will:
* Provide hands-on care as part of an interdisciplinary team, tailoring your approach to meet the unique needs of each resident.
* Deliver care in accordance with established policies and professional standards, always prioritizing resident dignity, independence, safety, and privacy.
* Administer medications and treatments, promptly reporting any changes in residents' conditions and initiating appropriate interventions.
* Collaborate with Registered Nurses (RNs) and the Director of Nursing (DON) to coordinate care and communicate effectively with families and healthcare providers.
* Maintain accurate clinical documentation, including care plans and medication administration records.
* Support the orientation and ongoing training of new staff, participating in performance management as needed.
* Supervise Certified Nursing Assistants (CNAs) and Certified Medication Aides (CMAs), ensuring high standards of care.
* Assign CNAs based on resident needs and overall care requirements.
What We’re Looking For
Qualifications:
* Education & Experience:
o Postsecondary certificate or Associate's Degree in Nursing from an accredited program required.
o Prior experience in long-term care or a similar healthcare environment preferred.
* Licensure & Certification:
o Current LPN license in good standing with the Minnesota Board of Nursing.
o Current CPR certification.
Skills & Abilities:
* Dependable and punctual with a strong attendance record.
* Excellent communication and problem-solving skills.
* Strong interpersonal skills to build positive relationships with residents, families, and coworkers.
* Organized, detail-oriented, and able to prioritize effectively.
* Knowledgeable about applicable healthcare regulations.
* Proficient with electronic medical records and Microsoft Office applications.
* Able to lead and supervise CNAs and CMAs effectively.
* Capable of understanding and following written and verbal instructions in English.
Physical Requirements
* Ability to sit at a computer and perform desk work for extended periods.
* Frequent standing, bending, walking, and movement required.
* Must be able to lift up to 50 pounds occasionally.
* Physical demands include stooping, pulling, pushing, grasping, and other repetitive motions.
