About the Service
The Children with Disabilities Service supports children and young people from birth to aged 18 and Care Leavers up to age 25 who have severe and profound disabilities, and their families. Working long term with families enables us to fully embrace and be on the journey with the children and young people we support and see their achievements at different stages of their life to ensure that they have good positive outcomes and that their voices are heard. We take a strengths‑based approach to supporting children and young people, to ensure we achieve the best for the children, young people and families we support.
Our service consists of 6 teams across Lancashire providing specialist social work for children and young people who meet the criteria for support, including assessments, direct work, safeguarding, court work, complex multi‑agency planning, and supporting transition to adulthood. Multi‑agency working is key to improving outcomes.
Responsibilities
As a Social Worker you will be involved in:
* Conducting assessments and developing care plans for children and young people with severe and profound disabilities.
* Providing direct support and overseeing the implementation of services for children and young people and their families.
* Managing safeguarding and court work, including complex multi‑agency planning.
* Working closely with multi‑agency partners to deliver joined‑up support.
* Supporting transition to adulthood for Care Leavers and ensuring continuity of care.
* Maintaining accurate records and reporting under the Mental Capacity Act and other statutory requirements.

Requirements
* You are a registered social worker with Social Work England.
* Ability to provide a car for duties; we may consider if you cannot drive because of a disability.
* Capacity to work 37 hours per week across Burnley & Pendle.
* Strong knowledge of safeguarding, the Mental Capacity Act and multi‑agency working.
* Excellent communication and interpersonal skills.
