Medicines Management Technician - Community Inpatient Services

NHFT is looking for a Medicines Management Pharmacy Technician to support the delivery of a clinical and technical pharmacy service to Spinneyfields inpatient unit within Northamptonshire Healthcare NHS Foundation Trust (NHFT).
Spinneyfields provides accommodation for people requiring health and social care recovery and rehabilitation, delivered through a multi-disciplinary team employed by Northamptonshire Healthcare NHS Foundation Trust (NHFT) in a recovering independence unit.
This is a varied role with the successful applicant needing to work independently and within the pharmacy team. They will have their own clinical responsibilities which will contribute to the healthcare team and support the pharmacist attending the wards. Travel between sites will be necessary and you must be able to travel independently to other bases in the Trust across Northamptonshire.
As a Medicines Management Pharmacy Technician, you will be required to work across the community inpatient team to support with medicines reconciliation, management of medication supply for in-patients and community patients including stock, non-stock, discharge and self-administration, participation in audit, support the use of the electronic prescribing system, and supporting ward areas with management of stock medications. You will also support self-administration where this is identified as appropriate by the multidisciplinary team (MDT).
Main duties of the job
o To provide medicines management pharmacy service to allocated in-patient areaso To work with the ward Pharmacists to identify new patients and complete the medicines reconciliation process using SystmOne, assessment of patients own medication and communication with patient/carer and/or by liaising with external organisations as appropriate.o To liaise with the ward pharmacists and ward staff over any relevant medicines management issues to ensure concordance/compliance with prescribed medication, highlighting any discrepancies where identified.o To support the MDT with assessing patients for self-administrationo To undertake patient counselling to selected patients and to work with the ward pharmacists to ensure patients understand their discharge medication.

Person Specification
Qualifications
* Current registration and membership with the GPhC as a Pharmacy Technician
* NVQ Level 3 in Pharmacy Services (or course equivalent )
* Evidence of recent continuing professional learning
* Previous use of SystmOne
* Previous use of electronic prescribing and medicines administration (EPMA) systems
Experience
* Post Qualification experience in the NHS as a pharmacy technician
* Knowledge of current medicines legislation
* Experience of Liaising with other healthcare professionals
* Membership of the APTUK
* Experience of working within a multidisciplinary team
* Experience of counselling patients on multiple medicines
