Job Overview

This is an exciting opportunity for a motivated and forward-thinking Band 6 Occupational Therapist to join our dynamic multidisciplinary team within Holywell Community Hospital setting. The role offers the chance to work with a diverse and complex caseload, supporting patients with a wide range of physical, functional and cognitive needs to achieve their goals as they transition through rehabilitation and recovery. The post is ideal for a skilled clinician seeking progression and looking to broaden their clinical skills in a supportive and collaborative environment.

You will play a key role in delivering high-quality, person-centered care, using your clinical expertise to promote independence, enhance wellbeing and enable safe discharge planning. The role includes supervision of a Rotational Band 5 Occupational Therapist, an Assistant Practitioner and students, making it an excellent opportunity for someone looking to consolidate or expand their leadership experience.

The ability to speak Welsh is desirable; Welsh and English speakers are equally welcome to apply.


Main Duties

* Provide comprehensive occupational therapy assessments for adults with varied and often complex needs, including physical, neurological and frailty-related conditions.
* Develop and implement personalised rehabilitation plans in collaboration with the wider multidisciplinary team.
* Lead on discharge planning focusing on a home first approach, working closely with patients, families and the wider MDT to ensure safe, timely and coordinated transitions of care.
* Use advanced clinical reasoning to identify risks, recommend appropriate equipment and adaptations, and advise on strategies to maximise safety and independence.
* Implement strategies to minimise de-conditioning and promote functional independence.
* Manage your time and caseload effectively, working flexibly to meet the demands of the service.
* Supervise, mentor and support junior staff, students and therapy assistants, contributing to their ongoing development.
* Participate in service development initiatives, quality improvement projects and audit activities to enhance clinical practice and patient outcomes.

Person Specification


Qualifications

* Degree or equivalent in Occupational Therapy.
* HCPC Registration.
* Post-graduate training / equivalent CPD in a specialist area.
* Experience of contributing to student placements.


Desirable Criteria

* Attended short courses at post-graduate diploma level.


Experience

* Significant post-graduate experience with demonstrable experience in physical health settings.
* Experience of contributing to service development and implementing service changes.
* Understanding of supervision and delegation principles.


Desirable Experience

* Broad band 5 rotational experience.
* Experience in community hospital OT service.


Aptitude And Abilities

* Ability to articulate the role of Occupational Therapy and implement the OT process.
* Ability to manage time and prioritise clinical and role demands.
* Ability to work independently and within a team.
* Communication skills and an ability to adapt these as situations dictate.


Desirable Abilities

* Ability to speak Welsh.
