Milford Hospital is a 34‑bed acute frailty unit, located near Godalming. Due to the hospital's location, the ability to drive would be beneficial.


The Role

With support from registered nurses and healthcare assistants, you will be responsible for delivering high‑quality, person‑centred care to individual patients or small groups of patients. You will work closely with the multidisciplinary team to promote safety, wellbeing, and meaningful engagement.

Key responsibilities include:

* Providing 1:1 enhanced observation for patients requiring additional support due to confusion, falls risk, or vulnerability
* Supporting and leading patient activities, including bingo, arts and crafts, lunch club sessions, and PAT dog visits
* Encouraging and motivating patients to participate in activities and social engagement
* Supporting and leading de‑conditioning prevention projects
* Promoting social interaction, music therapy, and therapeutic engagement
* Coordinating care and communication between teams, with support from your line manager, to assist in the provision of high‑quality care to patients and their families under the direction and supervision of the Multi‑Disciplinary Team
* Demonstrating a range of nursing skills to meet the needs of patients whilst promoting dignity, privacy and respect
* Complying with the Code of Conduct for Healthcare Support Workers and providing a range of health‑care and non‑nursing duties, ensuring an efficient and effective service to the ward/department at all times
* Providing nursing care under the guidance and supervision of a registered nurse and assisting with administration and non‑nursing duties at the discretion of the Ward Sister
* Helping to guide and develop band 2 HCAs whilst supporting the band 7 Line Manager, band 6 Junior Sisters/Charge Nurse and band 5 Staff Nurses; assisting the registered nurse in the assessment, planning and evaluation of nursing care as required
* Performing basic care under the direction of a registered nurse; assisting patients with daily personal hygiene, bathing/showering, eating and drinking, and caring for pressure areas, hair, nails and oral hygiene as required, reporting back to the nurse in charge any changes in the patient's condition
* Providing one‑to‑one care where deemed necessary and most required by the Nurse in Charge/Senior Sister/Matron (and above)
* Demonstrating an understanding of positive health and promoting positive health
* Assisting all patients who are able towards independence and activity under the direction of the nurse in charge
* Ensuring continuity of nursing care, updating the named nurse of any tasks, procedures and advice given to the patient and their family
* Ensuring that any entries in the nursing notes on Surrey Safe Care are countersigned by a registered nurse
* Maintaining confidentiality at all times
* Supporting the registered nursing staff in the clerical, administrative and receptionist duties on the ward
* Developing and utilising good communication skills and receiving and passing on telephone messages
* Assisting the registered nurse in the more technical aspects of nursing care such as dressings, observations, following a period of training and when deemed competent to do so by the Practice Development Team
* Ensuring that patients/clients belongings and valuables are kept safely, adhering to local procedures
* Supporting the patient and the carer by allowing them to express anxiety and stress during times of crisis, e.g. bereavement
* Being able to administer and seek advice on the appropriate first aid measures in an emergency
* Ensuring that patient/client privacy is maintained and that dignity and freedom of choice is enhanced
* Correctly using aids and equipment supplied for patients, reporting needs and obvious defects of this equipment to the nurse in charge; the Health Care Assistant is not responsible for the initial ordering of aids and equipment nor for their mechanical maintenance
* In the interests of health and safety, the HSN should only undertake manual handling of patients after having received adequate training and instruction
* Attending in‑service training, other training programmes and meetings, as required
* Participating in the induction of new Health Care Assistants (Nursing)
* Working within the current policies issued by the Trust, reporting any sickness and accidents to the nurse in charge and Senior Nurse
* Liaising with nursing colleagues and other professionals as appropriate
* Being aware of and complying with the Data Protection Act 1984
* Being required to use any computer system purchased by the Trust, with appropriate training given
* Participating in any other duties that may be asked by senior nursing staff
* Participating in the appraisal process for band 2 Health Care Assistants
* Collaborating with the ward team in the Achieving Excellence/Ward Accreditation programme
* Participating in teaching and supporting Health Care Assistants
