Independence at Home Senior Care and Support Worker Job reference: AHSS00381D3AGE Location: North Denbighshire (Rhyl and surrounding areas) Salary: Grade 7, £34,434 - £36,363 per annum Hours: 37 hours per week Contract: Permanent We are seeking a compassionate, motivated and experienced Senior Care & Support Worker to join our Independence at Home Team in North Denbighshire. This is a rewarding opportunity to support vulnerable adults to live independently in their own home while providing leadership and guidance to a committed team of Care & Support Workers. In this role, you will hold operational responsibility for the Lead Care & Support Worker at Hafan Deg Day Centre including providing regular supervision, planning and coordinating meaningful activity programmes and overseeing referrals, assessments and risk management. You will also play a key role in shaping and delivering high-quality, person-centred support to people in their own homes, ensuring services are safe, responsive and aligned with what matters most to citizens. Working directly with individuals, you will: - Build strong relationships to understand personal goals and strengths - Design and support effective care or reablement plans - Promote independence, wellbeing, choice and dignity at all times This post includes participation in an out-of-hours on-call rota, typically one week in every six, providing telephone support to staff. We are seeking someone with experience of supporting vulnerable adults, ideally within a community setting, who is confident in leading and motivating staff. Strong communication, organisational and IT skills are essential, along with a commitment to person-centred practice, safeguarding and high-quality care. You will need a recognised Level 3 Health & Social Care qualification (or equivalent) and be eligible to register with Social Care Wales upon appointment.
