Responsibilities
* Provide specialist Occupational Therapy assessment, treatment & review for patients with advanced palliative conditions or complex deteriorating neurology.
* Work as a Band 6 Occupational Therapist within the Community Specialist palliative care Team, collaborating with Occupational Therapists, Physiotherapists & Therapy Technicians.
* Deliver a comprehensive, evidence‑based assessment and treatment to adults in Coventry who have specialist palliative need in their own home or other community setting.
* Follow referral criteria to the Specialist Palliative Care Team.
* Liaise closely with team members and other community services to support patients' effective therapy.
* Provide advice & guidance to patients, carers and staff working in health & social care.
* Support professional development of staff within other services as appropriate.
* Contribute to service development and help meet key performance indicators.
* Assist in quality and activity monitoring of services.
* Adhere to the lone worker policy.
* Use specialist skills to manage a palliative care and neurological caseload in unstable or deteriorating phases of the condition.
* Adopt a patient‑centred, sensitive approach to therapy, collaborating with patients and/or carers in all aspects of the therapy process.
* Possess a high level of knowledge of a range of conditions (neurological, orthopaedic, musculoskeletal, elderly care, respiratory, falls) and understand the impact of rapid changes.
* Assist in quality and activity monitoring systems and support evaluation of service effectiveness.
* Communicate accurately and sensitively with patients, carers, consultants, GPs and other agencies.
* Participate in interdisciplinary meetings and other relevant meetings.
* Establish effective relationships with local voluntary and independent organisations to enhance care delivery.
* Ensure relevant information is communicated to other team members and agencies as required.
* Work collaboratively with other partnership organisations to ensure safe delivery and transfer of care.
* Serve as a source of information to MDT members.
Essential Criteria & Qualifications
* Band 6 Occupational Therapist qualification with experience in palliative care.
* Evidence‑based approach to assessment and intervention.
* Strong communication and motivational skills, able to discuss difficult diagnosis and prognosis with empathy.
* Ability to work independently and address consent issues where capacity is lacking.
* Excellent written and electronic communication skills for documentation and information sharing.
* Commitment to confidentiality and professional standards.
* Full UK driving licence (international licences not accepted).
* Valid immunisation and vaccination record from a UK occupational health provider or GP.
