We are seeking an experienced and motivated Registered Nurse, Occupational Therapist or Physiotherapist to lead our Community Transfer of Care Hub (CTCH). This is a pivotal Band 7 leadership role for a discharge specialist with expert knowledge in complex discharge planning, patient flow and system-wide working. You will lead and manage a team supporting patients with complex needs to transition safely from hospital into community placements, packages of care or community beds. The role requires strong clinical credibility, confident decision-making and the ability to work collaboratively across organisational and professional boundaries. The Ideal Candidate We are looking for a confident clinical leader who can manage complexity and influence across the wider system. Essential: RN Degree / Occupational Therapy / Physiotherapy qualification Active NMC or HCPC registration Significant post-registration clinical experience Experience in complex discharge planning Experience leading or managing staff Ability to manage conflict and make decisions under pressure Strong communication skills across multi-agency environments Experience initiating and implementing service change Desirable: Management qualification Teaching/assessing qualification Leadership programme completion Experience of financial management About The Company We change lives by transforming health and care.
