Become an important member of our Reablement Team and make a real difference to people's lives as they recover after a hospital stay. Our work is guided by empathy, respect, compassion and trust. We support individuals to regain confidence, rebuild skills and live as independently as possible through person centred and strengths based practice.
Responsibilities
- Complete assessments and design tailored reablement plans to guide individuals towards meaningful goals.
- Act as a trusted advisor within the team, offering guidance on equipment, safe practice and decision making.
- Manage your own schedule and provide personalised support across a varied caseload in homes, hospitals and community settings.
- Play an active part in supporting colleagues and contributing to continuous improvements across the service.
Qualifications
- Qualified Occupational Therapist.
- At least two years post-qualification experience.
- Full UK driving licence.
