We have a rare opportunity to welcome a new community support worker to join our established multi-disciplinary team as we continue to grow our service.
As part of the NHS Long Term plan, Lincolnshire was selected as one of only 12 areas to benefit from additional funding to transform community mental health services. The Community Rehabilitation Team is exploring new ways of working with people experiencing complex trauma, psychosis, and enduring mental health needs. The team has recently expanded its Lincoln and Gainsborough service to include Grantham and surrounding areas. As part of this exciting transformation, we are looking to increase our established multi-disciplinary team, which includes Community Support Workers, Occupational Therapists, Nurses, Social Workers, and a Psychiatrist.
We work in partnership with housing, CMHTs, and third-sector organizations to meet the recovery needs of individuals accessing our service.

Role responsibilities include working within policies, protocols, and clinical procedures of Lincolnshire Partnership NHS Foundation Trust, liaising with team members, promoting equality, understanding patient perspectives, maintaining accurate records, participating in supervision and training, and working flexibly across a 7-day service.
