Job overview

We are seeking an enthusiastic Band 6 Physiotherapist to join the Integrated Stroke Early Supported Discharge & Community Neuro Rehabilitation Service.

The Integrated service provided both stroke rehabilitation & community neuro rehabilitation supporting service users with a variety of complex needs.

This is an exciting opportunity to join an established multi-disciplinary team comprising of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants.

This role requires the successful candidate to manage a clinical caseload across the North of Cambridgeshire. Their base will be either Peterborough or Fenland dependent on location nearest to home.

The role involves visiting patients in their homes, wards, or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. We will provide a smart phone and laptop to support this role.


Main duties of the job

You will provide comprehensive assessments and deliver evidence-based specialist neuro Physiotherapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

The successful post holder must be able to travel independently across their work locality, have a valid driving licence and access to a car for work purposes.

Detailed job description and main responsibilities

Note: for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To be professionally and legally accountable for all aspects for your own and delegated work.

To work within Trust, CSP, and HCPC guidelines and to have a good working knowledge of national and local standards of practice in line with legal and operational requirements and use own professional judgement to vary from established practices where required.

To undertake holistic assessment of patients with neurological problems (including those with complex presentations and cognitive problems) making use of specialist clinical reasoning skills.

To use evidence-based practice, to ensure clinical care is effective and appropriate.

To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.

Organise and manage own time, delegating work appropriately.

Conduct risk assessments and health and safety assessments and including immediately reporting any changes/newly identified risks.

Person specification


Education / Qualifications

* Degree or equivalent, e.g. BSc, MSc or Diploma entry level qualification into the Physiotherapy profession.
* Current HCPC registration
* Documented evidence of own continuing professional development
* Willingness to undertake Fieldwork/Practice Educator qualification
* Member of CSP
* Member of ACPIN


Experience

* Relevant post-graduate clinical experience in a variety of settings.
* Experience of working in Neuro Rehab and post graduate training in the field of neuro rehab.
* Community experience
* Clinical educator experience


Skills & Abilities

* Able to demonstrate commitment to high quality care and service provision
* Ability to work flexibly as part of a team.
* Good communication skills and report writing including standard keyboard skills and ability to communicate through IT using packages such as Word.
* Able to assess plan and implement care
* Good time Management and prioritisation Skills.
* Experience of supporting unregistered staff including an understanding of clinical supervision.
* Accepts responsibility and accountability for own work and can define the responsibilities of others
* Recognises the limits of own authority within the role and seeks and uses professional support appropriately
* Understands the principle of confidentiality
* Demonstrates a knowledge of neuro rehab including cognition
* Knowledge of relevant outcome measures and experience using them
* Demonstrate proactive response to maintaining registration with HCPC and/or other professional bodies

* Understanding of prescribing equipment to facilitate and aid in activities of daily living


Physical Requirements

* The ability to kneel, bend & stoop, and work in cramped environments
* The ability to use moving and handling equipment to manoeuvre patients
* Must have access to a personal car and the ability to drive independently around the locality, due to the radius covered and lack of public transport in some areas
