An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards service as a Community Physiotherapist. The post will form part of a seven day service based within one of the Trust''s locality teams. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face-to-face visits and digital technology.
Responsibilities
* Work as a Physiotherapist to deliver a recommended and agreed treatment/care plan to service users in their own homes.
* Provide excellent assessment and intervention to plan, develop and evaluate goal-focused rehabilitation and reablement programs, as well as complex discharge planning.
* Collaborate with the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.
* Demonstrate high standards of evidence-based clinical practice.
* Prioritise workload, manage time effectively, and work flexibly within the team.
Qualifications and Skills
* Full and current registration as a Physiotherapist and relevant clinical experience.
* Excellent assessment and intervention skills; ability to plan and evaluate rehabilitation and reablement programs and complex discharge planning.
* Understanding of Health and Social care provision and ability to work with other organisations and stakeholders.
* Excellent interpersonal and communication skills; ability to work with and across teams.
* Strong commitment to values and behaviours; positive, can-do attitude.
