Job Purpose You will enable the people we support to lead fulfilling lives based upon their individualised support requirements. You need to work to the standards set internally (by Hft) and externally (for example, by the Care Quality Commission), ensuring that the people we support and your colleagues come to no harm and are treated with respect. You will achieve this for the people we support by communicating with individuals in the preferred communication style and by following the underpinning principles of Person-Centred Active Support (PCAS), namely: Making sure that people are supported in a way that gives them as much choice and control over their lives as possible which includes how they like to be supported Working alongside specialist teams within Hft and also externally to deliver a service model that promotes and reflects evidence-based practice for individuals with autism, challenging behaviour and/or complex health needs Following and supporting colleagues to implement the individualised support strategies put in place for the people we support Consulting with Hft or external professionals on a regular basis to agree any changes to support plans or positive behaviour strategies Ensuring that the sensory needs of the people you support are met by proactively working in line with the person s sensory support plan Keeping the people we support and yourself safe by following government and industry guidelines and wearing the correct PPE at all times Actively participating in training required to successfully carry out your responsibilities Showing demonstrable understanding and practice in relation to appropriate training Responsible for Key Accountabilities Support people with learning disabilities to the highest standard and in line with their support plan, cultural needs and wishes Carry out keyworker responsibilities including the review and updating of support plans, risk assessments and other relevant paperwork as required Create a positive sensory environment for the people you support and encourage them to engage in sensory activity Ensure that you follow positive behaviour or complex needs support plans, carry out observations and use the appropriate support monitoring tools Gather information for assessments of individuals to improve positive behaviour or complex needs support strategies Take part in regular feedback sessions with Hft or external professionals to update on an individual s progress/concerns Trained to appropriate PCAS level and complete individual and service observations as required Assist in goal setting and implementation of skills teaching ABC data gathering and formulation of charts for data analysis Attend multi-disciplinary/external team reviews Attend and actively participate in team meetings and 1-1 sessions, which may be outside of the hours on your rota and ensure actions are completed by the deadline agreed Support people with personal care if required, in line with their support plan Support and encourage people to make choices and life decisions in line with the Mental Capacity Act requirements Assist and encourage people to engage in everyday home, social, leisure and work-related activities, according to the individual s choice Support people with their health needs as required, e.g. taking medication and making and attending doctor s appointments Keep accurate and up to date records using internal IT systems Support people with their finances, where necessary, and in line with Hft guidelines Promote health, safety and well-being of the people we support Support people to manage their own home, maintaining their tenancies and give support and guidance when individuals move home Work positively with colleagues, families and external organisations and professionals. Look for opportunities to use Personalised Technology to increase people s independence Maintain confidentiality at all times Safeguarding - protect the people being supported from abuse/poor practice by undertaking Safeguarding Adults Training so you understand how, when and who to report concerns to Adhere to and maintain Hft s Health and Safety policies, standards and guidelines at all times Liaise with Hft s relief pool of staff and other core staff to cover shifts in the event of sickness and absence Be an active ambassador of Hft Carry out a range of duties and undertake any other duties as specified from time to time in accordance with Hft business requirements. It is the nature of the work that responsibilities are in many circumstances unpredictable and varied. All employees are therefore expected to work in a flexible way when the occasion arises in order that tasks which are not specifically covered in their job description are required. Professional & Technical Expertise Have a professional qualification in one of these sectors: vocational qualification (health, education or social care) social work qualification (Diploma, BSc, BA) teaching or education-based qualification nursing qualification (with current or lapsed registration) (NMC) other health professional qualification (with current HCPC or equivalent registration) Previous experience in supporting individuals with Autism is essential OR Significant experience in a similar field which can be demonstrated during the recruitment process.
