About the role: The Care Partner works in partnership with clients and the care team to co-design a plan of care, based on the client’s goals, preferences and assessed needs, whilst promoting functional independence, wellness and lifestyles choices. The Care Partner will liaise with internal and external stakeholders to ensure positive client outcomes within resource/budget allocations. What you bring: Certificate III Individual Support/Health Services, or equivalent relevant qualifications, or working towards or experience deemed equivalent by UPA Excellent communication and interpersonal skills Experience in the provision of Care Coordination, Care Planning and assessment expertise. Understanding of current Aged Care Legislation and Aged Care Quality Standards. Experience using a variety of computer programs including word, excel, email, internet, electronic client records). Current Drivers Licence. Criminal History Screening (Undertaken by UPA). First Aid (or willingness to obtain) COVID-19 vaccination and current Influenza vaccination.
