Groves Health is looking for an experienced and motivated part time Practice Nurse to deliver high quality care to patients in our four general practice sites across South West London and Surrey.
Working hours will be by arrangement and can be flexible.
Main duties of the job
Key responsibilities:
Conduct thorough assessments of patients' health conditions.
Formulate and implement care plans
Provide treatment as necessary
Perform treatment room duties and CDM clinics
Collaborate with other healthcare professionals to ensure optimal patient care.
Maintain accurate and up-to-date patient records
Offer health education and counselling to patients and their families
Stay current with the latest medical advancements and best practices
Qualifications
Current and valid registration with the Nursing and Midwifery Council (NMC)
Proven experience as an Practice Nurse
Child and adult immunisations, Cervical screening, Chronic Disease management, would care. Spirometry an advantage
Strong communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Commitment to providing high-quality patient care

Job Summary
1. To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
2. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, adult and child vaccination clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
Primary Responsibilities
3. Identify, manage and support patients at risk of developing long-term conditions
4. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
5. Provide vaccination and travel medicine services
6. Provide wound care (ulcer/ Doppler etc.) to patients
7. Request pathology services as necessary
8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary specialist care as required
9. Maintain accurate clinical records in conjunction with extant legislation
10. Ensure snomed codes are used effectively
11. Maintain chronic disease registers
12. Maintain vaccination programmes for adults and children
13. Provide cervical cytology
14. Priorities health issues and intervene appropriately
15. Recognise, assess and refer patients presenting with mental health needs
16. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
17. Support the clinical team with all safeguarding matters, in accordance with local and national policies
18. Deliver opportunistic health promotion where appropriate
19. Monitor and maintain baby immunisation targets
20. Monitor cervical smear results
21. Manage emergency drug kit
22. Attend clinical and practice meetings and clinical supervision
Secondary Responsibilities
23. Develop, implement and embed health promotion and well-being programmes
24. Implement and evaluate individual treatment plans for chronic disease patients
25. Assist GPs with procedures when required
26. Chaperone patients where necessary
27. Support the team in dealing with clinical emergencies
28. Liaise with external services/ agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
29. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
30. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
31. Act as the audit lead, effectively utilising the audit cycle
32. Monitor and ensure the safe storage, rotation, and disposal of medicines
33. Support junior members of the nursing team, providing guidance when necessary
34. Participate in local initiatives to enhanced service delivery and patient care
35. Support and participate in shared learning within the practice
36. Continually review clinical practices, responding to national policies and initiatives where appropriate
37. Participate in the review of significant and near-miss events applying a structured approach root cause analysis (RCA)

38. Nursing degree or equivalent experience Registered Nurse (NMC)
39. Evidence of immunisation training and assessment of competence in last 2 years
