Job summary

Would you like the opportunity to actively support and deliver effective patient care in GP Practices within a defined North Hertfordshire Primary Care Network? If so, you could be the qualified and experienced Clinical Pharmacist that we are hoping to welcome into our North Hertfordshire PCN (Icknield PCN) and work alongside our existing five PCN Clinical Pharmacists, three Pharmacy Technicians and six GP Practices' wider multi-disciplinary teams in Letchworth, Baldock and Ashwell.

We are seeking a highly motivated professional who will provide care delivered autonomously within their clinical competencies but from within a supportive multi-disciplinary team. They will provide expertise in clinical medicines optimisation across the Primary Care Network.

The role will suit a forward-thinking, innovative and enthusiastic Clinical Pharmacist who is able to work collaboratively across the team. If you are adaptable, self-motivated and organised, possessing the necessary qualities and skills for this post, we look forward to hearing from you.

Main duties of the job

* Provide professional input to PCN Practice teams & network multidisciplinary teams working to improve patient outcomes and reduce avoidable admission to hospital.
* Ensure optimum benefit for prescribed medication through high levels of compliance & concordance achieved by support for the patient and carers.
* Reconcile medicines following hospital discharge, amend medication lists & contact patients/ carers regarding such changes, as appropriate, to ensure patients receive the medicines they need post-discharge.
* Provide direct care to patients with long term conditions, minor ailments & other clinical areas within the scope of practice.
* Review MHRA alerts, organise appropriate searches/audits, implement drug withdrawals, etc; improving medicines safety.
* Support the care of complex patients in their own homes or in nursing or residential accommodation as part of a multidisciplinary team, with an option to undertake care home optimisation work.
* Provide professional leadership on matters relating to medicines optimisation and quality improvement.
* Lead on the prescribing domains of the Quality & Outcome and Consolidated Funding Frameworks.
* Act as key contact in Practice for all medication related queries.
* To support improvements in clinical care through Practice-based audits & implementing change.

12PointCare is the GP Federation supporting Icknield Primary Care Network (PCN) in North Hertfordshire with a patient population of circa 58,000.

All of our Practices operate the SystmOne clinical system, are well-respected, friendly and are well-rated by the CQC.


Person Specification


Qualifications

* Maintain registration with the General Pharmaceutical Council
* Relevant undergraduate degree in pharmacy (BPharm, MPharm or equivalent)
* Independent Prescriber or working towards/ intent of gaining qualification

Experience

* Interest in the development of the general practice clinical pharmacist role
* Portfolio of evidence of continuous professional development and post qualifying experience
* Evidence of multi-disciplinary working
* Evidence of the application of innovative practice and the application of evidence based interventions
* Clear, concise record keeping / report writing skills
* Evidence of multidisciplinary working
* Able to drive and has access to car or equivalent for travel between GP practices
* Good IT skills ability to use Word, Excel, PowerPoint and Internet to obtain and analyse information
* Ability to undertake research
