Primary Care Mental Health Practitioners (Mental Health Additional Roles Reimbursement Scheme) - Bradford City 4 Primary Care Network (PCN)
This role offers an exciting opportunity for a registered band 6 mental health professional (Nurse, Social Worker, or OT) to work across primary and secondary care, integrating mental health pathways and supporting understanding of mental health within primary care. Responsibilities include holistic assessments, linking individuals to community and VCSE resources, and facilitating seamless transitions into secondary care when necessary. The role involves participation in multidisciplinary meetings within GP practices.
The service is expanding as part of community mental health transformation, closely linked to the development of new models of support within primary and secondary mental health services. The successful candidate will be based at GP sites within the PCN during working hours.
Key responsibilities include:
1. Promoting understanding and knowledge of mental health and wellbeing within primary care.
2. Leading specialist assessments of service users with mental health issues within primary care.
3. Providing signposting based on holistic assessments, utilizing a range of resources including VCSE and community organizations.
4. Offering evidence-based, time-limited interventions within scope of practice.
5. Facilitating swift transitions into other mental health services such as MWBC, CMHT.
6. Collaborating with secondary mental health services to support effective service user transitions.
7. Working with service users and families to support shared decision-making, recovery, and self-care.
8. Contributing to the development of innovative support models within primary and secondary care.

Person specification
Experience
* Experience working with individuals with serious mental health problems.
* Experience working autonomously, assessing, developing care plans, and managing risk.
* Experience working collaboratively within an MDT.
* Experience in service development, quality improvement, and innovation.
* Experience in care coordination and signposting.
Qualifications
* Registered professional in Nursing (Mental Health), Social Work, or Occupational Therapy with current registration (NMC, Social Work England, or HCPC).
* Degree-level education or equivalent experience.
