Change Consulting are looking for a Homeless Pathway Officer to join a supported living provider based in Birmingham.
This is a permanent role, based full time on site. Travelling to other sites is required so a valid UK Driving Licence is desirable but not essential if you are happy to get around on public transport.
Experience Required:
* Experience working in a hospital environment supporting hospital discharge
* Experience supporting vulnerable adults - specifically the homeless
* Experience conducting needs and risk assessments for clients with complex needs
* Experience providing expertise on homeless legislation and housing rights
* Experience coordinating between housing, mental health, primary care and substance misuse services
* Proficiency in IT Systems (eg: Word, Excel, Outlook) and housing management databases
