About the Role
We are seeking a highly motivated and experienced Clinical Pharmacist to join a progressive and supportive GP group in Aylesbury. This is a full-time, patient-facing position across three well-integrated GP sites, offering the chance to work in a collaborative primary care setting.
You will play a key role in delivering high-quality pharmaceutical care, optimising medicines use, and supporting patient safety and wellbeing. Key Responsibilities
* Provide clinical pharmacy services, including medication reviews, medicines optimisation, and reconciliation
* Work collaboratively with GPs, nurses, and other healthcare professionals
* Support the management of long-term conditions and contribute to QOF and IIF targets
* Deliver patient education and counselling on safe and effective use of medicines
* Ensure compliance with relevant legislation, clinical guidelines, and best practice
* Assist in the development and implementation of prescribing policies and clinical protocols
Requirements
* Registered Pharmacist with the General Pharmaceutical Council (GPhC)
* 1–3 years’ experience in a Primary Care or General Practice setting (essential)
* Postgraduate qualification in pharmacy or clinical pharmacy (essential)
* Strong clinical knowledge and a patient-centred approach
* Excellent communication and interpersonal skills
* Ability to work both independently and as part of a multidisciplinary team
