We are looking for a caring, dynamic and highly motivated person to join the team as a Support Coordinator in our Chorley Supported Living Services. This is an exciting opportunity to be part of a well-established team that supports individuals with learning disabilities. You will have skills in positive communication and engagement and be able to demonstrate unconditional regard for the people we support, with the ability to assess needs and manage risk to enable active, fulfilling lives with a ‘can do’ approach.
Responsibilities
* Provide outcome-focused support to enable clients to lead fulfilling and valued lives, maintain their tenancies, participate in their local community, and more
* Meet the needs of clients and take additional responsibilities such as rota management, induction of staff, and staff supervision
* Work in a flexible manner, including evenings, weekends, and sleep-ins
* Participate in local On Call duties
* Administering medication
Qualifications and requirements
* Experience in positive communication and engagement with individuals living with learning disabilities
* Ability to assess needs and proactively manage risk to support effective service delivery
* Clean, up-to-date driving license and access to a vehicle for travel around the Chorley area
* Full training provided and opportunity to work towards QCF Diploma in Health and Social Care
