Registered Mental Health Nurse / Occupational Therapist
As a Registered Mental Health Nurse / Occupational Therapist, you will:
1. Undertake proactive transfer and case management activities for a defined group of individuals.
2. Ensure practice patients access suitable interventions for their needs.
3. Increase identification and management of the full range of adult mental health conditions in primary care, including initiatives to improve physical health outcomes for those with long-term serious mental health conditions.
4. Collaborate with community-based staff to improve detection of depression in people with long-term physical health conditions such as coronary heart disease, COPD, diabetes, or other clinical diseases as determined by CCG priorities.
5. Advise GPs on referral routes into local psychological therapies where indicated.
6. Provide time-limited emotional support to patients presenting with low to moderate risk, who do not require formal psychological or medical treatment at this time, e.g., social isolation.
7. Encourage the use of local open access community groups, such as those provided by religious organizations, Healthy Walk schemes, leisure centers, Citizens Advice Bureaux, etc., appropriate to individual needs.
8. Keep accurate records of activities undertaken for monitoring and evaluation purposes.
Clinical Responsibilities
* Assess and make evidence-based clinical decisions based on patient presentations.
* Manage a varied and complex caseload effectively, prioritizing clinical needs and time management.
* Facilitate or co-facilitate treatment groups in accordance with NICE guidelines.
* Administer and monitor depot medications within the depot clinic, advising and supervising junior staff.
* Monitor medication regimes, assess effects, and advise patients on safe storage and disposal of drugs.
* Undertake proactive case seeking and management activities for a defined group of individuals.
* Develop individualized management plans based on recovery principles using advanced clinical reasoning.
* Facilitate care transfer from secondary to primary care, providing transitional support as needed.
* Act as a key contact for individuals on the Mental Illness QOF register managed by primary care.
* Ensure annual physical health checks are performed and acted upon.
* Collaborate closely with secondary care for effective shared care and timely re-entry in case of relapse.
* Use clinical reasoning to prioritize patient appointments within the wider mental health services.
* Utilize assessment tools for comprehensive evaluation of patients with complex needs.
* Maintain knowledge of local community resources supporting psychosocial recovery.
* Supervise and oversee junior staff and students on placement.
* Maintain practice supervisor/practice assessor responsibilities.
