Main responsibilities

* Provide clinical leadership to our nursing teams to drive forward joined-up, equitable care within local communities.
* Work closely with Primary Care Networks (PCNs), voluntary sector partners, and residents to champion proactive, population-focused approaches, providing care closer to home.
* Lead on initiatives and plans across a range of issues, particularly continuous improvement in patient care delivery pathways and clinical quality.
* Provide assurance of proactive management of clinical and cost improvement plans, ensuring appropriate actions are taken to mitigate or respond to identified risks.
* Deliver a complex agenda which drives efficiency and quality initiatives across the partnership.
* Work in collaboration with the clinical leads for PCNs and Specialist services to deploy the nursing workforce effectively and maintain high standards of nursing care and intervention.
* Support, challenge and contribute to the performance and quality review process; provide professional nursing expertise and guidance for future progression of neighbourhood health services in Surrey Downs.
* Support and deputise for the Associate Director of Nursing as required.
* Collaborate with Operational Managers of the PCNs and Specialist Services to provide clinical guidance, supervision, and leadership across PCNs and Specialist Services.
* Assess and identify needs of clients with complex social and health care needs; participate in multidisciplinary teams to develop and deliver high quality services.
* Promote effective teamwork within Surrey Downs Health and Care and delegate activities to the wider team.
* Link closely with multiple other services and organisations as a Clinical Lead within Surrey Downs Health and Care.
* Achieve responsibilities through clinical practice, service development, education, training and research, with a focus on clinical safety, governance and quality.

Person specification


Qualification

* Current registration with NMC
* Postgraduate education or equivalent level (e.g., postgrad diploma) and evidence of CPD relevant to assessment and treatment of complex patients in hospital and in the community


Experience

* Recent postgraduate experience as a senior clinician within the community
* Experience working within a multi-disciplinary team
* Experience and evidence of leading and participating in service development
* Experience of supervision and teaching others
* IT skills including Excel, Word, PowerPoint, internet, email, and patient recording systems

* Valid driver’s licence with access to a car for work
